Health Insurance and National Health Insurance

Health insurance and national health insurance are public medical insurance systems of Japan. Insured individuals and their family members will be able to receive medical benefits and allowances when they become ill or injured. It is a system where each insured individual will pay a premium according to their income and everyone to bear each other’s medical expenses. It is different from a life insurance.

In Japan, everyone must subscribe to the public medical insurance and once subscribed, an insurance card will be issued. You must take this card with you when visiting a hospital. If you are working for a company, you will subscribe to a health insurance through your place of employment. Individuals that are self-employed, in farming business, or not subscribed to a health insurance at your place of employment will subscribe to the national health insurance.

National Health Insurance (Kokuho)

If you have completed the alien registration and have a residence permission of over one year and are not subscribed to the health insurance, you will be subscribing to the national health insurance. All family members of your household will be subscribing at the same time.

When under the following circumstances listed, follow procedures at the National Health Insurance and Pension Division (Kokuho-nenkin ka) in Central Office, or any Contact Service Centers.
* Depending on the nature of a procedure, the matter may be processed only at the Central Office.

Subscribing to a national health insurance

When discontinuing or terminating national health insurance

What is a national health insurance tax?

Highly important source of funds to cover for the medical expenses of all those subscribed to the national health insurance. Obligated taxpayer will be the head of the household, and tax rate of the national health insurance tax will be determined by income of subscribers and number of family members in the household (obligated taxpayer will be the head of the household, even if they are not subscribed to the national health insurance).

National health insurance tax will be determined in July, and the payment will be made in 9 installments from July (1st installment) through next March (9th installment). An insurance card with short period of validity will be issued, if the national health insurance tax is not being paid without any special circumstances. If there are difficulties in making the tax payment, please consult the national health insurance contact counter as soon as possible.

Please be aware

If the residence period is to expire within the fiscal year, insurance card must be renewed along with the residence period. Also, when there were changes made in the alien registration card of the national health insurance subscriber, please follow procedures for the insurance as well.
